We offer a holistic approach to chronic disease management. Patients will be sent an appointment around the time of their birthday. As far as possible, all diseases will be managed in the same appointment. However, respiratory diseases such as asthma and COPD (Chronic Obstructive Pulmonary Disease) may be monitored separately.

We are taking the Year of Care approach, so you will be invited in to see Jeni, our Healthcare Assistant, who will take your bloods and carry out some clinical screening such as height, weight, blood pressure, and foot check for diabetics. This will be a 20 minute appointment, you will then be given a further 20 minute appointment with Nickie, our Practice Nurse, to review your results and discuss your ongoing care with you using a patient-centred approach.

One of the most popular services we provide is with our Wellbeing Co-ordinator, Angela. She is in the practice on Wednesday only and is able to use her experience and expertise to have an in-depth conversation with any patient suffering from a problem that is, not necessarily, medical in nature.

Often, illness is a symptom of a much bigger personal problem and while a GP may be able to treat the physical symptoms they are not necessarily able to help with the greater issue.
