If you would like to register with Westminster Surgery, please complete a registration form and bring it into the surgery. You can find the registration forms below. Or, you can complete a registration form in surgery if you wish.

You will be asked to provide two forms of identification to show you live in the area (e.g. utility bill, bank statement) and a photo ID. You will then be invited to attend a new Patient Registration Health Check, which is carried out by our practice nurse or healthcare assistant. If you are receiving ongoing treatment please bring all the details with you.
We use an interpretation service (Language Line) to support people whose first language is not English.
Change of personal details
Please remember to let us know if you move or change your name or telephone number. Failure to do so could cause delays or prevent us from contacting you or a relative in an emergency.
