All babies require an 6-8 week health check which is carried out by our GP. This is a 20-minute appointment which will give you the opportunity to ask any questions or address any concerns regarding your baby’s health

We offer a holistic approach to chronic disease management. Patients will be sent an appointment around the time of their birthday. As far as possible, all diseases will be managed in the same appointment. However, respiratory diseases such as asthma and COPD (Chronic Obstructive Pulmonary Disease) may be monitored separately.

We are taking the Year of Care approach, so you will be invited in to see Jeni, our Healthcare Assistant, who will take your bloods and carry out some clinical screening such as height, weight, blood pressure, and foot check for diabetics. This will be a 20 minute appointment, you will then be given a further 20 minute appointment with Nickie, our Practice Nurse, to review your results and discuss your ongoing care with you using a patient-centred approach.

This is a 12 week programme and it is aimed at changing lifestyles and improving outcomes.

The first appointment is for 30 minutes and subsequent appointments last for 10 minutes. Patients are expected to attend every appointment and will be given a weight loss goal on week one.

This is usually set at 5% reduction from their starting weight. This is achieved through a combination of dietary and exercise advice, whilst medication may be offered where appropriate.

Bloods will be taken if necessary on week one and of course height, weight, BMI (body mass index) and waist circumference measured.

You may be invited for an appointment with Stephanie our Clinical Pharmacist. Stephanie may carry out a medication review with you and can answer any concerns you may have with regards to your current medication. She runs her clinic on Thursdays and Friday

This innovative service is designed to remove the long waiting times for referrals to the physiotherapy services at the Countess of Chester Hospital. One of the most popular services we provide is our Wellbeing Coordinator, Brian. He is in the practice Mondays, Wednesdays, and Thursdays and is able to use his experience and expertise to have an in-depth conversation with any patient suffering from a problem that is not necessarily medical in nature.

Often, illness is a symptom of a much bigger personal problem and while a GP may be able to treat the physical symptoms they are not necessarily able to help with the greater issue.
